All Students who play/plan to play Sports @ TCS for the 2022-2023 School Year are required to have an updated sports physical packet every school year. 

• Required for ALL Students playing sports (7th - 12th Grade) 
• MUST have ENTIRE packet in HS office before you can participate in any athletics beginning August 1st 
• Forms (A1-A6) are required medical forms and require a medical/Dr. Signature (from your family physician)
• Forms (B through F) require signatures please review all highlighted portions for signatures 

Please let us know if you have any questions - there are hard copy packets available in the HS office
